Cambridgeshire &Peterborough NHS Foundation Trust CPFT in partnership with UnitingCare Partnership UCP is implementing an exciting brand new model of care built around integrated neighbourhood teams of community, mental health, acute and social care staff. CPFT’s new Joint Emergency Team JET will play a vital
role in this model to help improve services for the people we serve.

CPFT is delighted to be part of the UCP family of organisations, which will be delivering cutting-edge integrated care for older people and adults with long term conditions in Cambridgeshire and Peterborough from the 1 April 2015, following UCP’s success in the largest ever NHS procurement process last year.
JET will care for people over 65 living in the community whose needs increase unexpectedly. The specialist team will provide emergency assessment and treatment and arrange care for older people round the clock, 365 days a year.
You may respond to falls, treat UTIs, put in place IV therapy or organise sitter support. You will also work as part of wider teams and could take part in the wider roster to meet service demands
