Role Summary:
This is an exciting new role combining providing clinical advice to patients via the NHS 111 pathways system and face to face clinical streaming within the Urgent Care Centres at PELC Sites

The role of the Clinical Advisor is to manage calls referred to them by a 111 Operator when clinical advice is required. The post holder will be responsible for the assessment of the patient’s health and clinical needs, utilising their professional judgement, supported by clinical decision support software.

The Clinical Streamer will be responsible for ensuring the safe and effective redirection of patients to the most appropriate clinician or heath care provider presenting to the Urgent Care Centres in accordance with approved guidance.

Key Responsibilities (advisor role)
To provide skilled and effective assessment of patients presenting clinical need, through telephone consultation, with the support of the clinical assessment software, throughout which the clinician is required to be accountable using professional judgement at all times.
To apply critical analysis to the synthesis of complex information during the care process to provide accurate advice and health information to patients in order to empower and enable them to act upon advice given.
To utilise advanced listening, probing and facilitative skills across a diverse range of calls, some of which are highly challenging due to emotive circumstances or caller aggression
Utilises complex communication skills to negotiate (utilising translation services in situations where language barriers are present), and provide support to callers who may be non-compliant with recommended outcomes, emotive, hostile and or antagonistic.
Liaises effectively between caller and third party in critical situations/areas of concern e.g. child abuse, where information is not consistent or may be disputed.
To keep immediate and accurate records of service user enquiries to the service during the consultation.
To be able to accurately document consultation notes in a timely fashion within the software programme, to allow for suitable emergency referral and intervention.
Identify and use information sources to support and underpin clinical decision-making.
Recognises opportunities for and provides health education during the consultation process, referring to appropriate health care professionals as required.
Use critical thinking skills to support call centre staff in decision making processes around complex/sensitive scenarios resulting in a safe user outcome.
Knowledge and competence in computer skills, in order to access ‘drives’, databases, email and approved internet resources and critically appraise sources of information and their evidence base.
Refers to supervisor/lead, peer or colleagues as necessary for advice, but maintains accountability and responsibility for decisions made in relation to clinical calls.
To act as a clinical resource for call centre staff.
Inform senior colleagues of any factors affecting the service delivery at the time an issue is highlighted.
To support line management through undertaking additional delegated administrative tasks when required to support effective service delivery e.g. entering faxed patient information from other sites onto software.
Develop and maintain expertise, practice and competence in the role of clinical/ nurse advisor.

Key Responsibilities (streaming role)
To take clinical responsibility for your decisions once fully orientated to the role and to identify and escalate any clinical governance issues.
To work as an autonomous Practitioner and provide a skilled and effective assessment of patients presenting to the Urgent Care Centre.
To provide direct access to patients requiring primary healthcare and to articulate to patients the rational of the clinical decision.
To demonstrate skilled, systematic decision-making to facilitate clinically safe destinations for patients to primary and secondary care.
Demonstrate the safe re-direction of patients to named clinicians or other health care providers for advice, support best placed to counsel patients on health and social matters relating to their health and well-being.
To share and utilise areas of specialist practice with peers and colleagues to inform and enhance patient care
To ensure that patients and carers who are at risk because of cultural and/or language barriers are helped to understand and make use of the various services available.
To act as an advocate for the patient and foster good relationships and collaboration within the health and social sector.
The post holder will act as a highly visible representative and ambassador for PELC within the EUCC and will ensure their profession is held in high regard.

Additional Information
Band 6 Equivalent salary scale plus 15% London Weighting.
Unsocial Hours, NHS Pension and 25 days A/L + B/Holidays.
